The COC HIPAA form Download COC HIPAA form states that you have been trained on HIPAA, the Health Insurance Portability and Accountability Act.  You haven't been yet, but we'll watch a video on it and cover it on our first day of class. (Something to look forward to!)

Instructions

• Step 1

• The forms do not download correctly from most phones. Students can access computers for free at the Valencia TLC Links to an external site. and Library Links to an external site. and the Canyon Country Library and Learning Center Links to an external site. in the Student Services Building. Students who have paid the Student Support Fee can print documents for free (worth up to $16/month ) through the Wepa app.
  
  • Download the form and save it 
  • Open the saved file (some of the boxes won't work unless you do this)
  • Type your information in the boxes

  • Print the form and use a paperclip to attach it to Checklist 1 Download Checklist 1 in the order shown on the checklist
