COC Waiver

Due Date: First Day of Class

The COC Waiver form Download COC Waiver form releases College of the Canyons from any liability if you are injured, become ill, or die as a result of class-related activities.  It is required for all healthcare courses.  Please read it carefully before signing.

Step 1

The forms do not download correctly from most phones. The ASG computer labs Links to an external site. are free, and have free printing

• Download the form and save it 
• Open the saved file (some of the boxes won't work unless you do this)
• Type your information in the boxes on BOTH pages

Step 2

Print the form and use a paperclip to attach it to Checklist 1 Download Checklist 1 in the order shown on the checklist. Do NOT use staples - you will need to upload this form into a website once class starts.
