Step 1: Fill out the Form

    • Have the doctor fill out and sign the rest
  • The physical can't be older than 6 months before the first day of class
  • You can go to your own doctor, or the Student Health and Wellness Center, or an urgent care. Costs will vary. 
  • Kaiser students - be aware it may take you 2-3 weeks after your appointment to get copies of your medical records.  Do your physical, bloodwork, and TB tests early.
  • If you are pregnant or become pregnant during this course, you must obtain a “release without restriction” from your OB to continue in the class. The OB must be made aware that the class will entail lifting, and possible exposure to communicable diseases and bloodborne pathogens, as well as riding in an ambulance. Without a release you cannot participate in skills or clinicals and will not be able to successfully complete the class. Even with a release our providers may decline to have you complete your clinical at their facility or ambulance, which will prevent you from completing the course. Late drops are available for medical reasons.

Step 2: Documentation

  • Add your physical form to your antibody test results, the completed Titers and Vaccination Form, vaccination record, the TB form, and the TB test results to Checklist 3 Download Checklist 3 in the order shown on the checklist
  • We DO NOT need your cholesterol levels, STD results, or any other lab results besides your antibodies for Hep B, MMR, and varicella. Please do not include these!!
  • Please remove all the staples from the documents before turning them in!
  • Do not check off any items on the list - your instructor will do that
